New patients can download the required paperwork from the website, or plan to show up 15-20 minutes early. Please note if you are a first time patient, I require 24 hour notice of change or cancellation or a $50 fee will be charged. Thank you for respecting the time I have reserved for your visit. For non-insurance patients, payment is expected at time of service in the form of cash, check, or credit card. Mastercard and Visa are accepted. The patient is fully responsible for the cost of the office visit and any medicinary items purchased. For patients using insurance, payment for all medicinary items is expected at the time of service.

I am available for office visits from from 9:30 a.m. - 5:30 p.m. on Tuesday, Thursday and Friday in downtown Seattle. I make house calls on Vashon Island Monday and Wednesday.
Payment
A note about health insurance:
I am contracted with Regence Blue Shield. For all other insurance plans I am an ‘out-of-network provider’ and payment is requested at the time of your visit. A service statement will be provided on request. Insurance covers “medically necessary care” and as such does not always cover the services I provide.
